How To Claim Your Credits
This program is approved from 2/13/2019 – 2/13/2021. Credit must be claimed prior the course expiration
To claim your credits, you will have to complete a post course test evaluation. The testing and accreditation of these courses is handled by FCMcme.org.
A login for FCMceme.org will be required.
To complete the course Post Test Evaluation and receive your Certificate of Credit, go to FCMcme.org and at the top click on “claim credit” choice and follow the instructions provided. The course code you will need for this event is WLW661020P6Y
For “new users” you will need to click on “new user” and create a login and password. When you have registered onto the site click on the “claim credit” prompt and enter the course code below.
Enter the program course code where indicated. You can find the program course code on your receipt of purchase for this course.
Now click on the “Take Exam” link which is displayed directly across from the program title. Click the link and follow the prompts.
Upon successful completion of the online Post Examination and Evaluation Form you will be directed to your FCM User Record page where you may Print/View your Statement of Credit (certificate).
Certificate links become a permanent part of you FCMcme.org user record. You may return anytime in the future to the FCMcme.org website to Print/View previously claimed credits.
Navigate to the FCMcme.org user login.
Return users will be directed to your FCM “User Record” page where a “Take Exam” link will be displayed directly across from the program title. Click the link and follow the prompts. If you do not see the specific course
exam you are looking for click on Claim Credit and enter the appropriate Course Code.

Conflict of Interest Disclosure Policy
It is the policy of the Foundation for Care Management (FCM) to ensure independence, balance, objectivity, scientific rigor and integrity in all of its continuing education activities. FCM requires everyone in a position to control the content of this activity to disclose any financial conflicts of interest they may have as related to the content of this activity and having occurred within the last 12 months. A Financial relationship are those relationships in which the individual benefits by receiving a salary, royalty, intellectual property rights, consulting fee, honoraria for promotional speakers’ bureau, has ownership interest or other financial benefit. The faculty must disclose to the participants any significant relationships with commercial interests whose products or devices may be mentioned. All identified conflicts of interest are thoroughly vetted by FCM for fair balance, scientific objectivity of studies mentioned in the material or used as the basis for content and appropriateness of patient care.
